Will Our Long-Term Care Facility Be Billed Anything For Your Services?
  -S.W.M. Direct Bills Medicare, Medicaid and Secondary Insurance.

Can Your Nurses See Hospice Patients?
  -YES.  As long as the patient's primary diagnosis is not wound related. Treating hospice patients becomes a quality of life and dignity issue and we may not treat their wounds as aggressively.

How Often Will You Be Treating Our Wound Patients?
  -Normally we see patients on a weekly basis. If medically necessary a patient may be seen twice a week.

Will S.W.M. Bring Physicians In To Assess and Treat Our Patients?
  -NO. All wound treatments are performed by masters prepared and debridement certified nurse practitioners. We do require a referral from the resident’s primary physician.

Will S.W.M. See Medicaid Only Patients?
  -YES, however the frequency of allowed patient visits varies from state to state.

Will S.W.M. Continue Care After a Resident Leaves Our Facility?
  -YES. We see many home health patients.

Does S.W.M. Provide The Products Needed For Your Treatments?
  -Our Nurse Practitioners will bring items incident to the treatments: debridement kits, gauze, and gloves, etc. We will work with your product vendors and will stay within your formularies. If our nurse believes a product that is outside your formularies would be more efficacious, the Nurse Practitioner will discuss this recommendation with the Director of Nursing.

What Do You Need From Us to Begin Treating a Resident?
  -Step 1: A copy of the facility's face sheet. If the patient is on hospice, please document that on the face sheet and provide the name of the hospice agency.
  -Step 2: A copy of the Physician's written/telephone order. The order should read: S.W.M. to evaluate and treat wound(s) as indicated.
  -Step 3: A verbal authorization from the patient's P.O.A. or a signed authorization on a Specialized Wound Management form.

Will Your Nurse PractitionersPerform Wound Rounds?
  -YES! We actually prefer to accompany your wound nurses on rounds when we open a new facility.
